Our Consent Forms are now available through our confidential Client Portal. After you have completed the intake interview with a member of our Triage Team, you will be given a link to establish a free account within our HIPAA-compliant Client Portal to submit your consent forms.
The Authorization to Disclose is an optional form you may complete and sign if you would like your therapist to consult with another member of your care team (i.e. your previous therapist, primary care physician, family member, etc.). You can download this form and provide it to your therapist.
